Invicta Athletics Program was founded in June 2017 under the DOS Crescent Foundation Inc. 501c3. The Invicta Athletic Program positively impacts youth in Pinellas County through the following sports: Cross Country, Track & Field, Basketball Ball and Flag Football. The objective of the program is to teach the value of teamwork through achievements of higher fitness levels and improved health. Demonstrable results through the redirection of child obesity, delinquent behavior and crime among youth in our community

Invicta Athletics Program is a community based health, fitness, behavioral modification, educational and character development group working with youths of ages 5 - 18 through the use of sports and fitness. Our programs promotes good health, fitness and wellness. The programs are designed to directly assist in the reduction of childhood obesity, The sports include: Cross Country, Track & Field, Flag Football and Basketball.

LIABILITY WAIVER

I/(WE), give permission for our child as named above to participate in the Invicta Athletics Program of the DOS CRESCENT FOUNDATION INC. In addition I/(we), the parent(s) of the above named youth do hereby authorize any treatment or emergency care needed for said child by any licensed nurse, physician, or hospital while participating in the activities of the Invicta Athletics Program of the DOS CRESCENT FOUNDATION INC.

As the parent(s) next of kin and guardian of said minor, I/(we), forever release, acquit and discharge Invicta Athletic program of the DOS CRESCENT FOUNDATION INC. from any and all liabilities, claims and I/(We) or my/(our) representatives may have by reason of said emergency care. My/(Our) child is covered by if under 18 this permission slip MUST BE SIGNED BY A PARENT OR LEGAL GUARDIAN.
